Download our Credit Application

At this time, we offer three options:

Option #1:
If  you have Microsoft® Word 97 for Windows® 95 or for Windows® 98, you can download this file: creditpro.doc.  This file is a form that can be filled in Word.  You can then save it on your hard drive, and e-mail it to us at: process@allegheny.com.
Please make sure to mail or fax a copy of the Credit Application with your signature.

Option #2:
Download the creditpro.pdf.  If you have Adobe® Acrobat® Reader, you can open the file, fill the form in Adobe, print the form, sign the form, and fax it back to us at (412) 749-9530.

If you do not have Adobe Acrobat Reader, click on the following icon to download it.

Option #3:
We can also fax you this application if this is more convenient for you.
